Fire Department Information Requests

Request incident reports where the City of Fort St. John Fire Department responded to an incident. 

You can request reports for:

  • Motor vehicle incident
  • Medical call
  • Structure fire
  • Motor vehicle fire

These requests for information are subject to the City's Fees and Charges Bylaw [PDF - 432 KB] and applicable legislation such as the Freedom of Information and Protection of Privacy Act.

Download and complete an Incident Request Form [PDF - 185 KB] and return it with payment to City Hall at 10631 - 100 Street between 8:30 am and 4:30, Monday through Friday.

If you are requesting access to another person's personal information, an Authorization to Release Form [PDF - 160 KB] must be completed and submitted with the request. If you do not provide a completed Authorization to Release form, any personal information will be removed from all records in accordance with the Freedom of Information and Protection of Privacy Act.


As these requests generate additional expenses for the City, they are provided to the public on a fee-for-service basis. The requested information may not be available. Fees are for research services and are non-refundable.
